Terms and Conditions Agreement

By purchasing this item, you acknowledge and agree to the terms and conditions outlined below:

Photo/Video Consent

I hereby grant permission to Jax City Ballet School  to use and reproduce any and all photographs, video recordings, or other media captured of my child during participation in classes, performances, or events. These images may be used for any lawful purpose, including but not limited to publicity, marketing, social media, print publications, or other promotional materials, without compensation. All such materials shall remain the sole property of Jax City Ballet School. I further waive any right to inspect or approve the finished product and release all rights to these images.

Release of Liability

I understand that participation in dance and related physical activities involves inherent risks of physical injury. By enrolling my child and/or making this purchase, I knowingly and voluntarily waive any and all claims, liabilities, or causes of action of any kind, including claims of negligence, that may arise from participation in such activities.

I agree to hold harmless and release Jax City Ballet School, its owners, instructors, employees, and affiliated personnel from any and all liability related to injuries or damages sustained while participating in programs, classes, or events.

I acknowledge that I am assuming all risks voluntarily and that this waiver is executed as a condition of participation. If the participant is a minor, I affirm that I am the parent or legal guardian and that I give my full consent for the child to participate. I further release Jax City Ballet School from all liabilities associated with my child’s involvement.
